Required Forms for all
Campers: The required forms below must be completed and
submitted by email by June 1, 2023. You can email your forms
to Youth.Programs@sdsmt.edu.
Did you know your phone is a scanner? You can print these forms, fill them out, scan them
into your phone, and email them to us. Where is your phone’s scanner? Apple phones:
Open up your notes app, click the write note picture, click on the camera, choose the scan
document option, take pictures of your documents, click save. Android phones: Download a
free scanning app, follow instructions, and capture your document. Now all you have to do
is email it to Youth.Programs@sdsmt.edu with your email on your phone.
